目的 采用决策曲线和剂量反应分析信号肽-CUB-表皮生长因子结构域包含蛋白1(signal peptide-CUB-epidermal growth factor-like domain-containing protein 1,SCUBE1)和血清纽蛋白(Vinculin)与老年心肌梗死(acute myocardial infarction, AMI)患者经皮冠状动脉介入治疗(percutaneous coronary intervention, PCI)术后心肌损伤的预测价值。方法 前瞻性选取2021年1月至2024年1月浙江中医药大学附属湖州市中医院重症医学科收治的老年AMI患者68例为观察组,并同期选择60例健康体检者为对照组;根据随访3个月后是否发生主要心血管不良事件(major adverse cardiovascular events, MACE)将观察组分为MACE组32例和未发生MACE组36例。检测患者血清SCUBE1、Vinculin含量、血清中肌钙蛋白I(troponin I,cTnI)、肌酸激酶同工酶(creatine kinase isoenzymes-MB,CK-MB)和肌红蛋白(myoglobin, Myo)。采用Pearson相关性分析SCUBE1、Vinculin与cTnI、CK-MB、Myo表达相关性。采用ROC曲线分析SCUBE及Vinculin对老年AMI患者PCI预后的预测价值。采用限制性立方样条图分析血清SCUBE1和Vinculin与老年AMI患者PCI术后发生MACE事件关联强度的剂量反应。采用决策曲线分析血清SCUBE1和Vinculin对老年AMI患者PCI术后发生MACE事件预测价值。结果 观察组cTnI、CK-MB、Myo、SCUBE1和Vinculin表达高于对照组,差异有统计学意义(P<0.01)。Pearson相关性分析结果显示,观察组SCUBE1、Vinculin与cTnI、CK-MB、Myo表达呈正相关;发生组血清SCUBE1和Vinculin水平高于未发生组;ROC曲线分析结果显示,血清SCUBE1和Vinculin预测老年AMI患者PCI术后MACE发生的曲线下面积为0.783、0.814,两者联合预测老年AMI患者PCI术后MACE发生的曲线下面积为0.959;血清SCUBE1和Vinculin连续变化与老年AMI患者PCI术后发生MACE事件呈非线性剂量反应关系(r=0.698,r=0.832,P<0.001),血清SCUBE1和Vinculin与老年AMI患者PCI术后发生MACE事件呈显著正相关,老年AMI患者PCI术后发生MACE概率随血清SCUBE1和Vinculin升高而升高;血清SCUBE1和Vinculin对老年AMI患者PCI术后发生MACE事件发生具有一定预测价值。结论 SCUBE1和Vinculin与老年AMI患者PCI术后心肌损伤发生存在相关性,且具有较好的预测价值。
